Graduate Forms/Resources

During your graduate studies many forms will need to be completed or obtained to allow progress through degree program completion. We have listed the most commonly used forms below. If you have any questions about these forms, please contact the Program Coordinator, Clarice Fulton.

Department Forms and Resources

ALEC  Graduate Student Travel Funding

Students participating in national or regional conferences or in an international experience study away/abroad may be eligible for departmental or college funding. The ALEC travel request deadline is at least 2 months prior to conference travel. HIE Scholarship application deadline for maymester, summer, and fall  international travel is February 1. Application deadline for wintermester and spring international travel is November 1. Applications for conference travel requests will be accepted until May 1 of each year.

The department encourages students to seek other funding opportunities through outside entities. A few are listed below:

Graduate and Professional School Forms

This office utilizes an online document system for students to submit Degree Plans and Petitions, click here to access the system for submission of the following:

Degree Plan submission
Change of Committee
Change of Major, Degree, or Department (MDD) – Must be submitted at least two months prior to the start of a semester
Change of Coursework
Waiver or Exception petition
Extension of Time Limits

Other commonly used Graduate School forms must be submitted utilizing DocuSign. IMPORTANT:  When completing DocuSign forms, all email entries must be listed as the “” email addresses. TAMU emails are provided in the ALEC Departmental Graduate Faculty listing OR access the TAMU Directory Web page.
